Not that I'm calling you an idiot, but I'm sure someone has, and without justification. It happens with other labels too. In fact, labels are usually the first defense of idiots.

Nobody, irl, has ever called me an idiot. I’m sure that doesn’t shock you at all.
Idiot is a more generalised term and the idiot caller will have their own interpretation of what makes someone an idiot.
Being called a racist/sexist/ homophobic etc is more specific and would be a result of something specific the recipient of the label has said or done, like name calling.
