It's a standard DWP questionairre that all claimants have to complete prior to their case being assessed for benefits.
It was first introduced in 2008 by the Labour government.

That headline should really say "Prove you're incapacitated". They don't go on what condition/disability someone has, they go on how that condition/disability affects someone. It could be assumed that a young man in that situation is not capable of doing any kind of work, but you never know I guess.

GP records should suffice, as the courts already go on those when presented at tribunal.

You don't even have to attend tribunals if you don't want to as they can be decided on paper based evidence as the judges are really pissed off with the government and these useless run for profit private rackets.
